THESE TERMS OF SERVICE ARE AVAILABLE TO ALL CLIENTS AND SHOULD BE UNDERSTOOD BY ALL STAFF, BOTH ADMINISTRATIVELY AND THERAPEUTICALLY. PLEASE TAKE THE TIME TO READ ALL OF OUR TERMS AND CONDITIONS, AS CLARITY IN COMMUNICATION AND UNDERSTANDING WILL ENSURE THAT ALL PARTIES ARE ON THE SAME PAGE AND ACHIEVING THE BEST POSSIBLE OUTCOME.
OUR PROFESSIONAL RECOMMENDATIONS ARE BASED ON THE INFORMATION PROVIDED BY THE CLIENT, AN OBJECTIVE AND EXPERT PHYSIOTHERAPY EXAMINATION AND THE SPECIFIC GOALS DETERMINED BY THE CLIENT AND THE PHYSIOTHERAPIST. IT IS IMPORTANT TO UNDERSTAND OUR TERMS AND CONDITIONS AND FOR THE CLIENT TO ACKNOWLEDGE THAT IT IS THEIR RESPONSIBILITY TO FOLLOW THE ADVICE AND AVOID SITUATIONS THAT MAY AGGRAVATE THEIR CONDITION. PHYSIO 15 ACKNOWLEDGE AN INFINITE NUMBER OF VARIABLES CAN IMPACT ON THE OUTCOMES OF ANY TREATMENT PROGRAM.
CONSENT
BY ATTENDING YOUR APPOINTMENT AT PHYSIO 15, YOU ARE PROVIDING IMPLIED CONSENT TO OUR THERAPISTS TO PERFORM ALL NECESSARY EXAMINATIONS, MANIPULATIONS, THERAPY, REHABILITATION, AND MEDICAL DIAGNOSTIC PROCEDURES IN ACCORDANCE WITH THEIR PROFESSIONAL TRAINING AND UNDERSTANDING OF YOUR INJURY. YOU UNDERSTAND THAT DURING YOUR TREATMENT, CARE MAY BE RENDERED BY DIFFERENT THERAPISTS AT STERLING PHYSIOTHERAPY. YOU ALSO UNDERSTAND THAT YOU HAVE AN OPPORTUNITY TO DISCUSS WITH YOUR PHYSIOTHERAPIST, THE NATURE AND PURPOSE OF YOUR PHYSIOTHERAPY CARE BEFORE ANY TREATMENT IS RENDERED. YOU HAVE THE RIGHT TO SEE THE PHYSIOTHERAPIST OF YOUR CHOICE, REFUSE INTERVENTION OR SEEK A FURTHER OPINION AND TO PROVIDE FEEDBACK AND MAKE A COMPLAINT. IN THE EVENT THAT THE OUTCOME IS NOT TO PLAN, THE CLIENT AGREES THAT SERVICE HAS STILL BEEN GIVEN, AND NO REFUND SHOULD APPLY.
PERSONAL INFORMATION HANDLING
PHYSIO 15 WILL COLLECT DETAILS FOR, BUT NOT LIMITED TO, TREATMENT DIAGNOSIS AND TREATMENT, MARKETING, PERSONAL COMMUNICATION AND ACCOUNT MANAGEMENT. BY ATTENDING A PHYSIO 15 APPOINTMENT AND PROVIDING THESE DETAILS, YOU GIVE CONSENT TO PHYSIO 15 TO USE THEM IN A RESPECTFUL MANNER, UNDER THE AUSTRALIAN PRIVACY PRINCIPLES, SPAM ACT AND PRIVACY ACT.
TO ACCELERATE THE MANAGEMENT OF YOUR INJURY, YOU ACCEPT IT MAY BE NECESSARY TO COMMUNICATE THE DETAILS OF YOUR MEDICAL CONDITION WITH TREATING PRACTITIONERS, REHABILITATION CONSULTANTS, CASE MANAGERS AND EMPLOYERS. BY AGREEING TO THESE TERMS OF SERVICE, YOU GIVE PERMISSION FOR REPRESENTATIVES OF PHYSIO 15 TO EXCHANGE INFORMATION REGARDING INJURIES SUSTAINED AND PAST MEDICAL HISTORY WITH THOSE DEEMED NECESSARY BY PHYSIO 15.
PRIVACY
PHYSIO 15 ADHERES TO THE AUSTRALIAN MEDICAL ASSOCIATION GUIDELINES ON PATIENT PRIVACY AND CONFIDENTIALITY.
COMMUNICATION
PHYSIO 15 MAY USE YOUR DETAILS TO BE IN CONTACT WITH YOU FOR APPOINTMENT REMINDERS, EMAILS OR LETTERS, MARKETING EMAILS OR NEWSLETTERS. YOU RESERVE THE RIGHT TO UNSUBSCRIBE, OR ALTERNATIVELY, YOU CAN CONTACT PHYSIO 15 TO BE TAKEN OFF THE CONTACT REGISTER.
PRICING
ALL PRICES ARE IN AUSTRALIAN DOLLARS AND ARE SUBJECT TO CHANGE WITHOUT NOTICE. PLEASE CONTACT PHYSIO 15 DIRECTLY FOR MORE INFORMATION ON PRICING. OCCASSIONALLY, THERE MAY BE DISCOUNTS THAT CERTAIN INDIVIDUALS ARE ENTITLED TO, INCLUDING, BUT NOT LIMITED TO PENSIONERS, LOW INCOME CARD HOLDERS AND COMMUNITY PARTNERS.
PAYMENT
PAYMENT IS TO BE MADE AT TIME OF CONSULTATION. PHYSIO 15 ACCEPTS VARIOUS FORMS OF PAYMENT (CASH, CHEQUE, CREDIT CARD AND PRIVATE INSURANCE CARD FROM APPROVED INSURERS). UNFORTUNATELY, PHYSIO 15 DOES NOT SUPPORT AMEX PAYMENTS. PHYSIO 15 RESERVE THE RIGHT TO CHARGE A CREDIT CARD FEE.
IN THE CHANCE THAT THERE IS AN OUTSTANDING PAYMENT ON AN APPOINTMENT, FURTHER APPOINTMENTS CANNOT BE ARRANGED UNTIL THE FEES HAVE BEEN PAID.
CANCELLATION AND LATE APPOINTMENTS
WHILST WE UNDERSTAND THAT SCHEDULES AND AVAILABILITIES MAY CHANGE DUE TO VARIOUS CIRCUMSTANCES, ALL APPOINTMENTS THAT ARE REQUIRING MOVING OR CANCELLATION SHOULD BE DONE SO 24 HRS IN ADVANCE. LAST-MINUTE CANCELLATIONS AND CLIENTS NOT ATTENDING THEIR SCHEDULED APPOINTMENTS PLACES A SIGNIFICANT IMPACT ON OUR OPERATING COSTS, WAITING LIST AS WELL AS NEGATIVELY AFFECTING THE TREATMENT PROGRESS OF THE CANCELLING CLIENT.
PHYSIO 15 RESERVES THE RIGHT TO CHARGE A CANCELLATION FEE OF NO MORE THAN THE COST OF THE APPOINTMENT. IF YOU ARE RUNNING LATE FOR AN APPOINTMENT, DEPENDING ON THE AVAILABILITY OF THE TREATING PHYSIOTHERAPIST, YOU MAY BE ONLY ABLE TO RECEIVE THE OUTSTANDING TIME OF YOUR APPOINTMENT FOR TREATMENT.
FOR NDIS BASED APPOINTMENTS, THE PARTICIPANT MUST GIVE 48 HOURS NOTICE IF THEY CANNOT ATTEND A SCHEDULED APPOINTMENT. IF THE PARTICIPANT FAILS TO GIVE 2 DAYS NOTICE THE SCHEDULED APPOINTMENT WILL BE BILLED FROM THE SUPPORTS ON THE PARTICIPANT’S NDIS PLAN.
GOODS AND SERVICES TAX (GST)
GST IS APPLIED TO RELEVANT SERVICES AND PRODUCTS AND WILL BE INCLUDED IN THE QUOTED FEE FOR SERVICE OF PRODUCT. MOST HEALTH CARE SERVICES ARE GST EXEMPT, EXCEPT IN CERTAIN CIRCUMSTANCES, WHEREBY THE SERVICE IS NOT FOR THE REHABILITATION OF AN INJURY. GOODS SUPPLIED WITH YOUR REHABILITATION MAY BE GST EXEMPT UNDER CERTAIN CONDITIONS.
COMPLAINTS
ALL CLIENTS HAVE VARIOUS AVENUES TO MAKE FORMAL AND INFORMAL COMPLAINTS. PLEASE CONTACT ADMIN@PHYSIO15.COM.AU, CALL PHYSIO 15 DIRECTLY, OR HAND IN A LETTER OF COMPLAINT TO PHYSIO 15 RECEPTION. ALL COMPLAINTS WILL BE HANDLED PROMPTLY AND YOU WILL BE NOTIFIED OF THE PROCESS AND OUTCOME.
EXCLUSION OF LIABILITY
THE DIRECTOR AND EMPLOYEES OF THIS PRACTICE SHALL NOT BE LIABLE FOR, NOR SHALL THEY ACCEPT ANY RESPONSIBILITY FOR ANY INJURY, LOSS, OR DAMAGE SUSTAINED BY ANY PERSON OR PERSONS ARISING OUT OF ANY OF THE TREATMENTS OR PROCEDURES DELIVERED DURING THE SERVICES OF PHYSIO 15 WHICH DOES NOT ARISE FROM ANY NEGLIGENT ACT OR OMISSION OF THE PRACTICE DIRECTOR AND EMPLOYEES.
TREATMENT PROGRAMS AND MANAGEMENT PLANS PRESCRIBED BY PHYSIOTHERAPISTS OF THIS PRACTICE ARE SPECIFIC TO THE NEEDS AND CIRCUMSTANCES OF THE CLIENT, AND MAY NOT BE APPLICABLE TO OTHER PARTIES. THE DIRECTOR AND EMPLOYEES OF THIS PRACTICE WILL ASSUME NO DUTY OF CARE IN RELATION TO USAGE OF A PRESCRIBED TREATMENT PROGRAM AND MANAGEMENT PLAN BY A THIRD PERSON OTHER THAN THE CLIENT.

- ‘NEW’ CLIENTS REFERS TO ANYONE THAT HAS NEVER RECEIVED SERVICES AT PHYSIO 15 BEFORE.
- THE OFFER APPLIES TO INITIAL AND STANDARD
- TO ENSURE THE BENEFIT OF THE OFFER IS PASSED ON TO YOU, PLEASE CONFIRM THAT YOU ARE CLAIMING THE OFFER WHEN BOOKING, OR WITH THE PRACTICE STAFF AT THE TIME OF ARRIVAL.
TITLE TO GOODS
GOODS PURCHASED AT PHYSIO 15 WILL ONLY BE TRANSFERRED TO THE CLIENT ONCE FULL PAYMENT HAS BEEN MADE.
AVAILABILITY OF PRODUCTS
PHYSIO 15 ONLY KEEPS LIMITED STOCK OF REHABILITATION PRODUCTS AND CANNOT GUARANTEE THAT ALL PRODUCTS ARE AVAILABLE IN CLINIC. OUR SUPPLIER DELIVERY CAN VARY FROM 24 HOURS TO 5 BUSINESS DAYS. WITH CERTAIN PRODUCTS, THIS TIMEFRAME MAY BE EXTENDED. ORDERING OF PRODUCTS MAY ONLY BE MADE WITH A DEPOSIT PAID AT TIME OF ORDERING.
RETURN OF GOODS
HAD A PRODUCT BE UNWANTED AND IS UNUSED, THE PRODUCT MAY BE RETURNED TO PHYSIO 15 WITHIN 7 DAYS. THE PRODUCT MUST BE IN ITS ORIGINAL PACKAGING. FOLLOWING THE 7 DAYS, IT IS AT THE DISCRETION OF THE THIRD-PARTY SUPPLIER AS TO WHETHER OR NOT THE REFUND MAY OCCUR. CLIENTS WILL PAY ANY ASSOCIATED SHIPPING COSTS TO RETURN THE PRODUCT TO THE SUPPLIER.
DAMAGED OR DEFECTIVE GOODS
IF ANY GOODS ARE DAMAGED OR DEFECTIVE, AND NOT THROUGH NORMAL WEAR AND TEAR, PHYSIO 15 WILL MAKE A JUDGEMENT ON THE GOODS AND REPLACE THEM AT NO COST TO THE CLIENT.
WARRANTY ON GOODS
ALL MANUFACTURERS WARRANTIES WILL BE UPHELD BY PHYSIO 15. WARRANTIES COVER PARTS AND MATERIALS COVERED IN THE MANUFACTURE OF THE GOODS, THEY DO NOT APPLY FROM IMPROPER USE OR OPERATOR ERROR. MAINTENANCE SHOULD BE CARRIED OUT REGULARLY TO ENSURE WARRANTIES REMAIN VALID.
